Volunteer Application

I understand, that depending on my role and responsibilities, I may be required to undergo a background check which will include a National Criminal Search (Standard), Sex Offender Search, SSN Trace, and Global Watchlist Search.
I understand that Ardent Solutions' is an employee at-will, and this policy includes volunteers.
I understand that I will not be paid for my services and that I am not eligible for employment benefits.
I understand that I will need to attend an orientation session and agree to participate in volunteer training as required.
I hereby authorize Ardent Solutions to verify all information contained in this application with any references, my past or present employers, any other appropriate personnel at my past or present employment, or other organizations where I have volunteered, and any individuals.
I authorize those who are contacted to disclose any and all information to Ardent Solutions. I release all such persons or entities from liability that may result or arise from Ardent Solutions collections of all such evaluations or information for its consideration of my application.
Should my application be accepted, I agree to follow the policies of Ardent Solutions and to refrain from inappropriate conduct in the performance of my services on behalf of the First Step Family Resource Center. I will review Ardent Solutions' values and agree to uphold them to the best of my abilities.
I understand that my personal information will be held confidential by Ardent Solutions' staff, and I am expected to uphold the confidentiality of those I serve, other volunteers, and Ardent Solutions' staff. I understand that the exception to this rule is if I suspect or witness child abuse of a child at the Family Resource Center. If such occasion occurs, I will immediately inform my supervisor and follow Ardent Solutions' policies and procedures regarding child abuse reporting.
