Full-Day Conference Objectives

Advance Care Planning assists patients in preparing for a sudden unexpected illness, from which (s)he expect to recover, as well as the dying process and ultimately death. Advance care planning is a gift to the patient and his/her family. It allows the patient to maintain control over how (s)he are treated and to ensure that (s)he experience the type of care and the type of death that desired.

Healthcare, legal and all community professionals have an opportunity and professional obligation to collaborate and make these preferred practices a reality in New York State. To do so, participants will gain a better understanding of the importance of how to:

1.Define value of MOLST, a key pillar of palliative care

2.Review advance care planning and the difference between advance directives and medical orders

3.Discuss the clinical process, the ethical framework, and the shared, informed medical decision-making process for making MOLST decisions

4.Recognize the ethical framework for making decisions to WH/WD life-sustaining treatment and legal requirements in NY, with or without MOLST

5.Explain how eMOLST improves quality and patient safety, reduces harm and achieves the triple aim

Distinguished Presenter: Dr. Patricia Bomba

Dr. Patricia Bomba is a nationally-recognized expert who currently serves as the Vice President and Medical Director, Geriatrics for Excellus Health Plan, Inc. and subsidiaries of The Lifetime Healthcare Companies. She is a nationally recognized palliative care and end-of-life expert who designs and oversees the implementation of community projects. Prior to her work at Excellus Health Plan, Inc., she was engaged in private practice in Internal Medicine and Geriatrics in Rochester, New York.

Dr. Bomba is New York State’s representative on the National POLST Paradigm Task Force, a multistate collaborative. In addition to serving as a New York State Delegate to the White House Conference on Aging, she served as a member of the Review Committee of the National Quality Forum’s “Framework and Preferred Practices for a Palliative and Hospice Care Quality” project, chairs the BlueCross and BlueShield Association National Medical Management Forum and is a member of the Medical Society of the State of New York Ethics Committee.
